How can I make an appointment?

Your GP will need to send a referral letter to the room in order for us to make an appointment for you. Alternatively you can contact Dr Gopinath’s room for appointment and bring a GP referral letter on the day of appointment.

What should I bring for my first appointment?

It will be helpful for you to bring:

  • Your Medicare card, health fund information or concession card

  • Any relevant imaging results (ultrasound scan, X-Ray, CT scan or MRI) blood test and urine test report

  • A list of current medications that you are taking

  • A list of known drug allergies and the symptoms you may have from taking these medications

  • Completed bladder diary (Download form)

  • Completed pelvic floor questionnaire (Download form)

Will my medical records be kept private?

Our practice respects privacy and confidentiality of patient’s health information. The contents of your health records will not be released without your consent. When necessary, the records will be shared with other medical providers to ensure continuity of care.

Does my private health fund cover the consultation?

Your private health fund only covers the hospital costs. It does not cover outpatient costs, but you are entitled some rebates from Medicare that can be claimed on the same day. If you are with Veteran Affairs, they will cover the costs of consultation. However if you require a pessary, Veteran Affairs will not cover the cost.

How can I make a payment?

We accept cash payment, EFTPOS, Visa or Mastercard. 

 

Pelvic Floor Questionnaire

This questionnaire contains questions about your pelvic floor function. Completing this questionnaire will help us in understanding your concerns so that we can help you. You will also be asked to complete this questionnaire after treatment so that we can assess the change due to treatment.

Patient Registration

Please complete your details online via the link provided by our reception staff when you were contacted for booking the appointment. Alternatively, download this form for completion. This form can be filled electronically and sent to our email address info@cairnsurogyn.com.au or brought with you during your clinic appointment. Completing this form prior to your visit, will save time on the day of your appointment.

Will my medical records be kept private?

You will be given the option to electronically access your health records of your visit with Dr Gopinath. Please let the reception staff know if you wish to access this when you check in or select this option using the online link at the time of initial booking.

Bladder Diary

A bladder diary is a simple chart, which allows you to record your daily fluid intake and urine you pass throughout the day and night. This information will be valuable for your doctor when treating your bladder problems and also monitoring the treatment effects.
